Job Title: Director of Patient Services

Position Overview
The Director of Patient Services (DPS) is a key leadership role responsible for overseeing the clinical operations of a licensed home care services agency (LHCSA). The DPS ensures compliance with all state and federal regulations, leads clinical teams, and drives operational excellence. The ideal candidate will bring hands-on leadership experience in managing home care services with a strong focus on team leadership, regulatory readiness, and quality patient outcomes.
Core Responsibilities
Regulatory Compliance & Quality OversightEnsure compliance with NYSDOH LHCSA and CHHA regulations, Medicaid/Medicare standards, and applicable federal laws.
Lead and prepare for all audits, including full recertification audits; draft and execute Plans of Correction.
Maintain licensure, certifications, and agency accreditations.
Develop, revise, and implement policies and SOPs to ensure quality and operational efficiency.
Monitor and support quality assurance and performance improvement (QAPI) initiatives.
Oversee intake, clinical appropriateness, and all aspects of patient care delivery.
Coordinate with interdisciplinary teams to ensure comprehensive, high-quality care plans.
Address and resolve patient complaints promptly, maintaining high satisfaction levels.
Apply a proactive, problem-solving approach to care coordination and case management.
Direct and mentor a clinical team of at least five or more staff members.
Conduct performance evaluations and ensure ongoing staff development and training.
Foster a collaborative and accountable clinical culture, ensuring high standards of care.
Lead clinical case conferences, supervisory visits, and regular team meetings.
Develop and manage clinical budgets and resource allocation.
Collaborate with finance, billing, and reimbursement teams for proper coding and billing accuracy.
Oversee staffing levels, clinical workflows, and service coverage.
Manage vendor relationships and medical supply inventory.
Serve as the clinical liaison to families, physicians, and community partners.
Represent the agency at professional meetings and in discussions with regulatory bodies.
Required Qualifications
Bachelor's or Master's in Nursing (BSN/MSN) or a related healthcare field
Active RN license in New York State (willing to obtain NJ license if not already held)
Minimum of 5 years of LHCSA/CHHA leadership experience
Proven ability to manage clinical teams (minimum of 5 direct reports)
Strong background in developing SOPs, managing audits, and budget oversight
Exceptional customer service and communication skills
Proficient in EMR systems; familiarity with HHAeXchange and/or Kantime a plus
Prior experience with JCAHO standards preferred
Participation in at least one full DOH recertification audit required
Preferred Qualifications
Experience in PDN or ICU settings
Case management, business development, or sales experience
Experience with acquisitions or launching new service lines
Work Environment & Expectations
Office-based with travel required to patient homes, hospitals, community events, and field operations
Must be available for on-call responsibilities as needed
